Spontaneous bacterial peritonitis.

چکیده
Spontaneous bacterial peritonitis (SBP) is a common, serious complication in patients with cirrhosis and ascites. It is a bacterial infection of ascitic fluid in the absence of an adjacent, surgically amenable infection source. Since its description in 1964 (1) numerous studies, guidelines, and both national and international consensus conferences have brought significant advancement to the diagnosis and treatment of this condition, and significantly changed its prognosis (2-12). Initially considered a fatal complication of cirrhosis, with mortality above 90%, the condition may be currently treated with an in-hospital mortality rate around 2030% (13). However, it still is a highly relevant condition in clinical practice given its high prevalence (it affects 10-30% of inpatients with cirrhosis and ascites), high recurrence rate (up to 70% in the first year), and poor longterm prognosis with mortality reaching 50-70% at one year, which leads to consider patients recovering from SBP potential candidates for liver transplantation (4-10). Spontaneous bacterial peritonitis in extrahepatic portal venous obstruction
Spontaneous bacterial peritonitis is defined by a positive ascitic fluid bacterial culture and an elevated ascitic fluid absolute polymorphonuclear count (≥250 cells/mm3) without an evident intra-abdominal, surgically treatable source of infection. Transient ascites is well documented in patients with extrahepatic portal venous obstruction but spontaneous bacterial peritonitis complicating extr...

Intestine in Liver Cirrhosis
Spontaneous bacterial peritonitis is a common illness in patients with cirrhosis and ascites that occurs without any apparent focus of infection. Bacterial translocation plays an important role in spontaneous bacterial peritonitis and it is evident from a variety of studies that the gut is a major source of this bacteria. Gut motility alterations, along with bacterial overgrowth and changes in ...
متن کاملStudy on ascitic fluid protein level in cirrhotic patients with spontaneous bacterial peritonitis.
Complement 3 is an important component of ascitic fluid total protein, which offers local defence against infection of the ascitic fluid. Hepatic synthesis of complement 3 as well as other proteins and their concentrations in ascitic fluid is significantly reduced in patients with advanced cirrhosis. The level of protein in ascitic fluid in cirrhotic patients with spontaneous bacterial peritoni...
متن کاملSpontaneous Bacterial Peritonitis due to Ochrobactrum anthropi: A Case Report
We report a case of spontaneous bacterial peritonitis from Ochrobactrum anthropi. O. anthropi is recognized as an emerging pathogen in immunocompromised patients. In contrast to most previously described cases, the patient reported here had no indwelling catheter. To our knowledge, no case of O. anthropi spontaneous bacterial peritonitis has been reported in the medical literature until now.
متن کاملNontypeable Haemophilus influenzae as a cause of spontaneous bacterial peritonitis.
Haemophilus influenzae rarely causes spontaneous bacterial peritonitis. We describe a typical case of spontaneous bacterial peritonitis in which the causative organism was identified as nontypeable H. influenzae, biotype III. Infection progressed despite the presence of adequate serum bactericidal antibody, probably due to the absence of complement in ascites fluid.
